I (the reader and duly authorised person) hereby apply for the creation of peering sessions with the ExchangeNET route servers in accordance with the current ExchangeNET Terms and Conditions , the CityLink Terms and Conditions, and the current Schedule of Services and Charges. I declare that I know of no debt owing to CityLink Ltd in respect of previous failure to pay and I agree to pay for all applicable services and connections as set out in the current CityLink Schedule of Services and Charges.

Installation and monthly fees will be confirmed by CityLink on receipt of this form.
